Job Description:


Performs complex case management work. Work involves reviewing and tracking case management activities; and assisting in developing program goals, objectives, and procedures. May assign and/or supervise the work of others. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.

Essential Functions

* Maintains a working knowledge of community resources; * Coordinates referrals to community agencies and tracks outcomes of referrals; * Completes client assessments and care plans by identifying psychosocial, economic and physical needs of clients; * Documents all client encounters; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the client; * Reviews encounters and records of community prevention workers; * Identifies problem areas, service gaps, and areas that are barriers to services in the Lubbock community; * Coordinates organizational and community groups that are working to address social determinants of health; * Develop and implement educational programs; * Assists with the development of policy and procedures; * May supervise or coordinate the work of community workers; * Plan goals and objectives for assigned programs; * Assist in budget preparation; * Assist with health department social media.


Experience in case management or social services work. Graduation from an accredited four-year college or university with major course work in social work, behavioral sciences, social sciences, public health or related field is preferred.

Knowledge and Abilities

Knowledge and Abilities:

* Knowledge of local community resources for social service, mental health and healthcare; * Skill in developing, implementing and monitoring service and treatment plans; * Knowledge of current principles and practices of planning and program implementation; * Knowledge of organization and administration of public health and/or human service programs; * Knowledge of program monitoring and policy analysis; * Skill in communicating effectively orally and in writing; * Skill in interviewing; * Skill in organizing and developing programs independently; * Skill in establishing effective and constructive working relationships; * Skill in use of personal computers and related software applications, including e-mail; * Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community;

Physical Requirements

* Occasionally stand and walk; * Frequently flex upper truck forward; * Occasionally lift up to 10 pounds; * Occasionally push and pull objects; * Occasionally flex upper trunk forward, partial flexing o knees; * Occasionally reach above, at or below shoulder height; * Continuously talk, see and hear.
