**Covenant Health is seeking a Pharmacy Resident at Covenant Medical Center.**

**Job Summary:**

The Pharmacy Practice Residency Program is designed to provide experience & training in hospital pharmacy practice at Covenant Medical Center. Under the primary direction and supervision of the Residency Program Director, and general direction of the Director of Pharmacy, the hospital pharmacy resident is an integral member of the health care team, responds to all drug-related needs of patients, nursing staff, medical staff and other health care professionals at Covenant Medical Center by performing or overseeing those professional activities of pharmaceutical care, drug compounding, distribution and provision in his/her assigned area. These activities are performed within the context of established policies, procedures and techniques of Covenant Medical Center, the CMC Pharmacy Department, and federal and state standards and statutes.

The resident position is one of high visibility as a representative of the Department of Pharmacy and he/she is expected to uphold the goals and objectives of the department. In many ways, the pharmacy resident serves as a liaison between the Pharmacy Department and other health care providers at CMC. Active participation in departmental activities is expected as the resident is integrated into a unique role in the department.

Each resident will actively work with the Residency Program Director to develop an individualized program plan with specific goals which will be unique to that resident. Specific responsibilities are required which include: 4-6 week rotations of a clinical or administrative nature, completion of special projects and assignments and scheduled work as a staff pharmacist.

**Essential Functions:**

Customer Service: Maintains and promotes positive, open and honest interpersonal relationships with customers (physicians, employees, visitors, patients, and other CMC publics). Provides high quality service to all customers; focuses on understanding customers' needs and follows through. Demonstrates respect for the capabilities, different cultures and or personalities of co-workers and Medical Center publics within the mission/values of CMC.

Communication: Promotes and demonstrates effective, open and honest written and verbal communication skills.

Self-Management: Demonstrates self-management behaviors which reflect a positive image of CMC, following established policies and procedures and promote an efficient, cost effective work environment.

Ownership /Stewardship and Accountability: Demonstrates good stewardship of resources by treating property of customers, Covenant Medical Center and co-workers with care and respect.

Teamwork: Works cooperatively and collaboratively with all levels of staff and other disciplines for problem solving without blaming.

Flexibility: Demonstrates flexibility by readily adapting to different situations to complete work.

Assessment: Assesses and documents sub-optimal pharmacotherapy of the patient.

Pharmacist Diagnosis: Analyze assessment date in order to formulate legible written statements of a patient's existing or potential drug related problems.

Planning: Utilize assessment date and pharmacist diagnosis to design, promote and legibly document a drug therapy plan consistent with identified patient problems.

Implementation: Promotes and monitors the medical/pharmacy therapeutic plan that will produce specific therapeutic outcomes for the patient.

Drug Distribution: Monitors all elements of daily dispensing, inventory, housekeeping and security according to all regular agencies and policies and procedures to ensure quality of services.

Evaluation: Monitors and documents legibly, accurately and completely all components of the drug therapy plan.

Leadership: Utilizes principles of leadership in accomplishing patient drug therapy goals and in performing leadership functions to guide, direct, supervise and coordinate staff to efficiently achieve and maintain an orderly pharmacy unit (satellite/station) in an allotted time frame.
