Job Description:

Encompass Health Corporation is seeking a dedicated and compassionate Case Manager to join our dynamic team, working in a challenging yet rewarding environment focused on patient-centered care. Our Case Managers play a pivotal role in facilitating and ensuring seamless transitions for patients through various stages of care within our network of inpatient rehabilitation hospitals, home health, and hospice services. By joining us, you will contribute to our mission of setting the standard in post-acute care delivery with an emphasis on high-quality, cost-effective healthcare. Assess patient care needs and develop individualized plans of care in collaboration with healthcare team members. Facilitate and coordinate all aspects of patient care from admission to discharge. Act as a patient advocate, ensuring access to required services and supporting patients and their families in decision-making processes. Regularly evaluate patient progress and adjust care plans as necessary. Communicate effectively with patients, families, and interdisciplinary healthcare teams to ensure understanding and compliance with treatment plans. Manage resources efficiently to reduce waste and unnecessary delays. Prepare and maintain accurate patient records, submitting all necessary documentation in a timely manner. Strong organizational and communication skills. Ability to handle sensitive information confidentially. Experience with electronic health record (EHR) systems. Knowledge of healthcare regulations and accreditation standards. Ability to work effectively in a team and independently. Medical Insurance Dental Insurance Vision Insurance 401k Certified Case Manager (CCM) preferred. Valid state nursing license or social work certification.
