Job Description:

are currently seeking a dedicated and compassionate Case Manager to
join our skilled team of healthcare professionals. The Case Manager
will be pivotal in planning, facilitating, and advocating for
patient needs to ensure optimal healthcare outcomes. This position

needs and develop individualized care plans in collaboration with
healthcare providers. Coordinate with physicians, nurses, and other
healthcare professionals to ensure comprehensive delivery of
services. Advocate for patients' needs and rights for appropriate
services, both within and outside of the healthcare facility.
Monitor patient progress and adjust care plans as necessary.
Conduct patient and family education on healthcare processes,
illnesses, and treatment plans. Manage a caseload and prioritize
needs effectively, maintaining detailed and confidential records.
Strong organizational and multi-tasking skills. Excellent
interpersonal and communication skills. Proficiency in healthcare
management systems and patient care software. Ability to work
collaboratively in a multidisciplinary team environment. Medical
Insurance Dental Insurance Vision Insurance 401k Certified Case
Manager (CCM) preferred. Valid nursing or social worker
license.
