Job Description:
Encompass Health Corporation, a leader in integrated healthcare
services, is seeking a dedicated and compassionate Case Manager to
join our dynamic team. This position is vital in providing top-tier
care coordination and support within our diverse network of
inpatient rehabilitation hospitals and home health agencies. The
Case Manager will be instrumental in creating effective treatment
plans and ensuring seamless healthcare services, thus directly
impacting patient outcomes positively. Assess new patients,
reviewing their medical history and healthcare needs. Develop,
implement, and evaluate individualized patient care plans.
Coordinate multidisciplinary healthcare services from admission to
discharge. Act as the primary point of contact for patients,
families, and healthcare providers. Monitor patient progress and
make adjustments to care plans as necessary. Ensure compliance with
healthcare regulations and internal policies. Facilitate patient
access to appropriate healthcare services, including specialty
referrals and community resources. Document all patient
interactions and interventions accurately and comprehensively.
Strong interpersonal and communication skills Proficient in
healthcare management systems and electronic health records Ability
to work collaboratively in a multidisciplinary team Critical
thinking and problem-solving skills Strong organizational and
time-management skills Medical Insurance Dental Insurance Vision
Insurance 401k Licensed Registered Nurse (RN) or Licensed Social
Worker (LSW) preferred. Certification in Case Management (CCM) is
highly desirable.
