Job Description:
Community Options, Inc.is a national non-profit agency providing
services to individuals with disabilities in 10 states. We believe

experience the highest degree of self-determination.We are seeking
an experienced Licensed Vocational Nurse (LVN) to deliver care to
individuals with disabilities in a community setting, utilizing the
nursing process of assessment, planning, intervention,
implementation, and evaluation. The LVN effectively interacts with
the patient, significant others, and other health team members
while maintaining standards of professional nursing.Pay Range: $23
- $26 per hourJob Responsibilities:Provide treatment and health
care procedures ordered by a physician and required by standards of
professional practice or law.Participate in quarterly and Annual
Individual Plan (IP) meetings for assigned participants.Create and
distribute Individual Service Plan Medical/Health Reports detailing
participants' general health, health care goals, and physician
recommendations for each participant.Provide documentation
regarding individual's health status for various support
people.Monitor participant's use of medications through Medication
Administration logs.Make referrals for needed medical services and
ensures that follow-up care is pursued.Provide pre-service training
for all support staff on consumer specific medical needs.Accompany
individuals to and from community, social, and recreational
activities.Advise supervisor of all concerns and/or issues related
to the provision of nursing services.Assist in building a support
network between the individual, family members and the
community.Follow strict OSHA procedures regarding blood borne
pathogens.Participate in Circle(s) of Support for personal futures
planning.Cooperates with internal and State inspections and
investigations.Knowledge/Skills Required:Bachelor's Degree in
related field preferred.Minimum 1 year of experience providing
nursing services to individuals with disabilities.Valid State
Nursing License.Understanding and commitment to community based
support for persons with disabilities.Ability to calculate figures
related to medication dosages.Ability to establish a comfortable
and supportive relationship with a person receiving supports.Valid
Driver's License.Working Conditions:Ability to lift 50 pounds
(weight of average manual wheelchair when chair is empty).Frequent
lifting, stretching and other physical exertion during positioning
of consumer's or equipment.May be on call, via cell phone, 24 hours
a day for medical emergencies, in cooperation with other management
staff.May transport participants in own vehicle.May perform tasks
that involve exposure to various medical conditions, communicable
diseases, blood, body fluids, or tissues.Work will take place in
the administrative office, community, and in the homes.What We
