We seek a skilled Care Manager to develop and coordinate comprehensive care plans for individuals with significant mental/behavioral health needs, ensuring quality, cost-effective healthcare outcomes.

**Job Description**

You could be the one who makes a meaningful difference in the lives of our members. As a Care Manager, you will work with diverse populations, evaluating their needs and developing personalized care plans to address complex mental/behavioral health challenges.

**Key Responsibilities:**

* Evaluate the needs of high-risk members with mental/behavioral health needs and recommend effective care plans
* Act as a liaison and advocate between members, physicians, and facilities/agencies
* Support members with mental/behavioral health needs, such as those with major depression, bipolar disorders, or substance use disorder
* Perform frequent home and/or site visits to assess member needs and collaborate with resources
* Provide education to members and their families/caregivers on topics such as preventive care, procedures, and healthcare benefits
* Coordinate community resources, including medical, behavioral, and social services
* Ensure accurate documentation and compliance with clinical guidelines and state/federal regulations

**Requirements:**

* Master's degree in Mental Health or Social Work or Graduate from an Accredited School of Nursing
* 2-4 years of related experience
* Licensed Behavioral Health Professional or RN with psychiatric experience
* Strong clinical documentation skills and ability to work independently with minimal supervision
* Proficient computer skills with Microsoft and video conferencing applications

**Preferred Experience:**

* Master's level un-restricted licensed behavioral health clinician
* 3+ years of case management experience with adult populations with serious mental illness and behavioral health conditions
* Experience working in in-patient/out-patient facilities, community-based Behavioral Health, or state Mental Health Authorities settings
* Experience administering psychological assessments and managing high-volume caseloads
