Job Description:
A company is looking for a Registered Dietitian, Care Manager
for a 100% remote position.
Key Responsibilities:
Consult and educate members, families, and care team on nutritional
needs and care plans
Ensure compliance with applicable regulations and document member
interactions
Collaborate with care management team to evaluate and provide
effective services
Required Qualifications:
Registered Dietitian certification
Experience in assessing nutritional needs and developing care
plans
Knowledge of NYS regulations related to nutrition
Ability to collaborate with a multidisciplinary care team
Experience in documenting member interactions and care plans
